Many people feel that the most natural and comfortable place to be at the end of life is at home, supported by family and friends.
Nearly 90 percent of hospice care is provided in our patients’ homes, or the place they call home (like a nursing home). Together, with you and your loved ones, we help you stay at home by developing an individualized care plan that is specific to your unique needs and wishes. We teach your family how to care for you, and we provide scheduled, routine visits by your care team.
We also act as your “safety net” in case you need additional support. This means that, in addition to scheduled visits, we are available 24 hours a day, seven days a week, in person or on the phone, should a need arise.
