What is Palliative Care?

Palliative care is medical care that focuses on providing relief from the symptoms and stress of a serious illness with the goal of improving quality of life. Palliative care addresses emotional, physical, and spiritual needs, as well as the needs of the family, while following your set goals and wishes for care. Palliative Care can be offered along with curative or other treatments you may be receiving.

We offer home-based palliative care that is tailored to each person’s needs, so it looks a little different for each one. After receiving a physician referral, a PalliaCare Cincinnati nurse practitioner will make a home visit to determine a plan of care. You and your care team will determine the frequency of future visits.

Who can benefit from PalliaCare®?

PalliaCare is for anyone living with a serious illness, including heart and liver failure, lung and kidney disease, and neurological conditions, such as ALS, Parkinson’s and Multiple Sclerosis, and cancer.

Who is my PalliaCare® team?

Your PalliaCare team includes specially trained nurse practitioners in consultation with your medical provider and a multidisciplinary team of nurse coordinators, social workers, and Palliative Medicine physician oversight. A registered nurse with expertise in symptom management is available by phone 24/7.

Who pays for PalliaCare®?

PalliaCare Cincinnati is billed to your insurance company just like a doctor’s visit. Co-pays may apply.

Palliative Care includes:

A palliative care plan may include one or more of the following goals:

  • Manage symptoms, such as shortness of breath, anxiety, weight loss, pain, etc.
  • Ease treatment side effects, such as fatigue, nausea or constipation
  • Address social, financial or spiritual issues that are affecting you and your family
  • Cope with feelings of depression, anxiety or grief
  • Explain treatment options and help make decisions that align with values and goals
  • Help with forms that are related to important medical decisions and care choices including living wills, health care power of attorney and emergency health care plans
  • Identify and access community resources to support you throughout your illness
  • Find strategies to help you adapt to physical and life changes that can accompany serious illness

Home-Based Palliative Care

PalliaCare Cincinnati is home-based, so we come to you. Care is provided in the home, assisted living and independent living facilities. Palliative care may also support patients who are being discharged to their home from a rehabilitation facility and have complex care or symptom management needs.

While receiving PalliaCare you may continue:

• To see your regular doctors and specialists

• To receive aggressive, curative treatment for your disease

• To use home health services including skilled nursing and aides, and physical and occupational therapy

Hospital-Based Palliative Care

You, your family members or your physician can request a consult for Palliative Medicine while you are in the hospital to provide an extra layer of support and focus on your goals, wishes and preferences for medical care.
