HOC’s Advanced Dementia Care Program provides the hospice care you need in the place you call home, surrounded by the people and things you love most.

Our goal is to improve the quality of life for all those living with, and caring for people living with dementia, through enhanced caregiver education, symptom management, and maintaining dignity.

How is the Advanced Dementia Care Program different than other hospice services?

To be eligible for this program, you must meet eligibility for hospice care (a diagnosis of a serious, terminal illness with a life expectancy of six months or less). Once enrolled in the Advanced Dementia Care Program, patients will receive additional benefits such as:

  • Frequent home visits and calls by a nurse who has received specialized training in dementia care
  • A care team that includes trained dementia care specialists
  • Dementia specific algorithms and care plans to address and manage unique needs of advanced dementia patients

How do I know if this program is right for me?

Patients can be eligible for hospice care when Dementia is either a primary or secondary hospice diagnosis
