How is the Advanced Dementia Care Program different than other hospice services?
To be eligible for this program, you must meet eligibility for hospice care (a diagnosis of a serious, terminal illness with a life expectancy of six months or less). Once enrolled in the Advanced Dementia Care Program, patients will receive additional benefits such as:
How do I know if this program is right for me?
Patients can be eligible for hospice care when Dementia is either a primary or secondary hospice diagnosis
- If primary diagnosis, the patient must meet eligibility criteria for hospice care
- Patients can be enrolled in the Advanced Dementia Care Program (ADCP), in addition to other specialty programs

All patients enrolled in the HOC Advanced Dementia Care Program will receive the Patient and Caregiver Booklet which contains the best standards of care for advanced dementia patients.
