How is the Advanced Dementia Care Program different than other hospice services?
To be eligible for this program, you must meet eligibility for hospice care (a diagnosis of a serious, terminal illness with a life expectancy of six months or less). Once enrolled in the Advanced Dementia Care Program, patients will receive additional benefits such as:
How do I know if this program is right for me?
Patients can be eligible for hospice care when Dementia is either a primary or secondary hospice diagnosis
- If primary diagnosis, the patient must meet eligibility criteria for hospice care
- Patients can be enrolled in the Advanced Dementia Care Program (ADCP), in addition to other specialty programs
